What this form collects
An appointment enquiry records your name, email, chosen treatment, optional preferred date and optional acquisition source. It also records your contact consent, notice version and the date the enquiry was saved. Do not submit health information.
Why the details are saved
These details are stored to allow the business owner to review and respond to your appointment interest. Giving contact consent for an enquiry is not permission for marketing. An enquiry does not create a confirmed appointment.
Who can access enquiries
Public visitors cannot browse, edit or delete enquiry records. Access to stored enquiries is restricted to the app’s administrators. The website uses Base44 to store the submitted records.
Policy details still required
The owner must complete their data-controller identity and contact details, applicable lawful bases, retention periods, processor and transfer details, and the procedure for access, correction, export and deletion requests before public launch.
Your rights & questions
The full policy must explain how to exercise applicable data rights and how to contact the business. The owner’s contact details are not yet supplied. Please do not submit an enquiry if you are not comfortable doing so before those details are available.
Before public launch
This notice is not a completed legal policy. Final privacy documents, any consent requirements and data-retention arrangements need appropriate review. Secure client management and consultation handling are not yet enabled.
